EXEMPTIONS:

* HOMESTEAD EXEMPTION: Click here to file for Homestead Exemption online Note: requires Internet Explorer 5.0 or newer.

* HOMESTEAD: Declaration of Domicile (required for permanent resident alien and non-voter applicants).

* HOMESTEAD: Certificate of Trust (required for some applicants when property is held by a Trust). Note: This form is only required if submitted in lieu of a copy of the actual, executed trust agreement.

* HOMESTEAD: Voluntary Cancellation of Homestead Exemption.

* SENIOR'S HOMESTEAD: Senior Citizen Homestead Exemption (Form DR-501SC).

* SENIOR'S HOMESTEAD: IRS Form 4506 - Required for some Senior Homestead Exemptions.

* DISABILITY EXEMPTION: Physician's Certification of Total and Permanent Disability (Form DR-416).

* DISABILITY EXEMPTION: Statement of Gross Income (Form DR-501A). Note: Only required for certain individuals seeking the total exemption from Ad Valorem taxation.

* $5,000 VETERAN'S DISABILITY EXEMPTION: No specific application form required. Submit a copy of your Certificate of Disability from the U.S. Government or the U.S. Department of Veterans Affairs (or predecessor agency). Must also provide proof of being a permanent resident of the State of Florida.

* HOMESTEAD PORTABILITY: Application to Transfer Homestead Assessment Difference (Form DR-501T) . IMPORTANT: This form is an application to transfer your Save Our Homes benefit to a new homesteaded property. You MUST also file for a homestead exemption on the new property.

* "GRANNY FLAT": Application for Assessment Reduction for New Construction for Parent(s) or Grandparent(s).

* HISTORIC PROPERTIES: Application for Special Exemption for Designated Historic Properties.

* RELIGIOUS, NON-PROFIT, CHARITABLE, SCIENTIFIC & EDUCATIONAL EXEMPTIONS: Application for Ad Valorem Tax Exemption (Form DR-504).

* NON-PROFIT EXEMPTIONS: Affidavit for Nursing Homes and Homes for Special Services (Form DR-504S).

* NON-PROFIT EXEMPTIONS: Exemption Application for Charter School Facilities (Form DR-504CS).

REAL PROPERTY (ALL):

* CHANGE OF MAILING ADDRESS (For use with both Exemptions & Real Property).

* NOTICE OF OWNERSHIP CHANGE: Change of Ownership or Control of Non-Homesteaded Property (Form DR-430) . IMPORTANT: Per Section 193.1556, Florida Statutes, you must notify the Property Appraiser in writing when you obtain ownership (or controlling interest in a business owning) of non-homesteaded property. You are NOT required to file any notice IF you have recorded a deed documenting the change of ownership.

* INCOME & EXPENSE SURVEY for COMMERCIAL Properties.

* INCOME & EXPENSE SURVEY for APARTMENT Buildings.

* MOBILE HOMES: Declaration of Mobile Home as Real Property (Form DR-402).

AGRICULTURAL CLASSIFICATION:

* AGRICULTURE: Application for Agricultural Classification (Form DR-482).

TANGIBLE PERSONAL PROPERTY (TPP):

* TPP: TPP Exemption Application and Personal Property Tax Return (Form DR-405).

* TPP: TPP Exemption Application and Return for previous filers with total TPP value of under $25,000 (Form DR-405EZ).

* TPP: Name & Address Change.

* TPP: Request for 30-Day Extension of Filing Date. Per state law, this request must be submitted in a reasonable amount of time BEFORE the annual April 1 TPP filing deadline.

VALUE ISSUES: The VAB will accept 2010 value petitions starting when the TRIM Notices are mailed in August 2010. ALL 2010 VALUATION PETITIONS MUST BE RECEIVED AT THE VALUE ADJUSTMENT BOARD OFFICE BY THE SEPTEMBER 20, 2010 DEADLINE. NOTE: Anyone seeking to file a 2010 value appeal after the September 20 deadline must contact the VAB directly (see above) for details on "good cause" late filing.

EXEMPTIONS: All applications for 2010 EXEMPTIONS should be filed WITH THE PROPERTY APPRAISER by the March 1, 2010 timely filing deadline (but, in no instance, be filed after the September 20, 2010 TRIM Notice appeal deadline). You are only required to file a VAB petition for an exemption when you are appealing a denial or removal of exemption by the Property Appraiser.

* Petition Withdrawal Form - This form is used when a petitioner, for whatever reason, simply wishes to withdraw his/her appeal.

PETITIONS TO CHALLENGE 2010 VALUE ASSESSMENTS:
You may file a petition to challenge your proposed 2010 assessment any time after the TRIM Notices are mailed in mid-August. The statutory deadline to timely file a 2010 value petition is September 20, 2010. If you miss the September 20 deadline, contact the VAB directly (see above) for details on "good cause" late filing for a value appeal.

* Petition to Challenge Value Assessments and Value Good Cause Request (DR-486 Form) - Note: This form is used for valuation challenges for both real property and tangible personal property. Check the appropriate box for either "Real Property Value" or "Tangible Personal Property Value." This form is also used to request GOOD CAUSE to late file if you miss the September 20, 2010, deadline. Contact the VAB directly for more information if you wish to learn more about "good cause" late filing.

* Contiguous Parcels (Multiple Condo Unit in the Same Condo)

* Petition Withdrawal Form - This form is used when a petitioner, for whatever reason, simply wishes to withdraw his/her appeal.
